Step 7: Reporting, Completion and Extensions

Reporting Quick Facts

Don’t forget to gather data for your evaluation throughout the life of your project.

You must submit a final report to be eligible to apply for future SMCF grants

COMPLETION

Every approved grant recipient must submit a final report. At the end of the 12-month grant period, you have 30 days to submit your grant final report.

Please read the most current Grantee Final Report Form to get the full list of questions.

Using our templates you will need to provide:

  • Descriptions of achievements and outcomes
  • Actual financial and budget information
  • If your project is going to continue, how you plan to finance it
  • Environmental projects will need to provide information about environmental impact.

All projects must provide basic quantitative data. You will need to tell us:

  • how many people your project served
  • how many adults and children
  • the counties of residence for participants/clients
  • client ethnicity.
  • If your project was funded in the Educational Category we will also need numbers of people served broken down by school ages.

Diversity is highly valued by the Spirit Mountain Community Fund and we understand that developing diversity is an ongoing process. You will also be asked if your organization's board diversity has changed or grown and if you have read Spirit Mountain Community Fund’s Diversity Statement.

EXTENSIONS

Occasionally unavoidable circumstances delay projects. If your project is going to exceed the 12-month period allowed by Spirit Mountain Community Fund, you must contact our Director by email to request an extension.
Please contact our Director at least 30 days before the end of your grant period. Some extensions require approval by our Board of Trustees and it is vital that you give us enough time to consider your request.

The e-mail should include:

  1. The reason for the extension request
  2. Any changes to the project scope or budget
  3. The time needed (cannot exceed 6 months)
  4. Any other information that may be relevant to the extension request. Please answer these questions, but the request does not need to be lengthy. 

After the Director (and Board of Trustees where necessary) have considered your request a staff member will send you our decision in writing.
